why do my consent form need a request for access to be granted

I didn't realize that there is some setting that is not allowing my patients to access the consent form...

I changed the setting to submit and edit but these setting explanations are not clear and nothing says anything about me needing to AUTHORIZE access to these forms.

As you have a HIPAA-Enabled account, I am unable to check your account myself.

I assume that you have set the settings of the form to Private under the PUBLISH tab? For Private forms, you will need to invite respondents via email only: when you invite them via email does the form still ask for authorization?

Please let us know if the issue persists? If you'd like to enable people to fill out the form without needing authorization access or to invite via email, you may need to publish the form as PUBLIC.

Is the form in question https://www.jotform.com/210415276890052? I am able to access it via the direct link without issue. If not, could you please confirm the Form URL?
