Is there a way to add an "N/A" option to the rating scale element?

Unfortunately, no, it is not currently possible to insert any additional option to the Scale Rating field other than the numbers. This field is only meant to collect rating scores, it is not meant to collect any other type of data.

Instead, what you can so do is insert an additional field like a single or multiple choice field next to the rating field and ask the user to click if they want to add a rating. If the answer is "yes" you can show the rating field, but if the answer is "no" then the field can be hidden. This way the user will not be forced to fill in the rating field.
