Format of the Email Notification is incorrect

    Hello!

    I need some assistance with the results of my form coming back incorrect. I have a form that has options hidden, but when the option to show both is chosen, the email it send me is correct, and then one or the other is chosen, the email that gets sent to me is incorrect. Can I please have some assistance with correct this so that the format that it sends me is correct?

    I checked your emails, and it seems correct to me. Can you please share a screenshot of a correct and incorrect email? It will help us further investigate the issue.

    Thanks for getting back to us. I have attached a few different screenshots to further explain.
    A. - Regular Protein option selected. This is pretty much what it should look like. There is a missing '/' in one of the Double Protein lines, but that's not a huge deal, since the only selections on that form are for the Regular Protein entrees. However, if we could figure out why it shows like that, it would be helpful.
    B. - Both Regular Protein & Double Protein options selected. This is perfect, and exactly what the full form and responses should look like. I have a conditional format that hides Double Protein when "I only have a Regular Protein plan" and one that hides Regular Protein when "I only have a Double Protein plan" selected. This is when someone selects "I have both plans".
    C. This is when someone selects "I only have a Double Protein plan", which ends up messing us the entire formula. I do not know why it doesn't show any numbers or information.
    Let me know what you think and how we can fix this. I am trying to make a form that we can send to our clients to choose their meals each week. The menu changes every single week, so we are wanting a form that will populate the results and format it in this style way to eliminate a step for our team. If it's easier to hop on a call to troubleshoot this, please let me know as we are wanting to get this rolled out sooner than later.

    You are getting the email from the third screenshot because the values of the hidden fields are cleared on submit. To resolve this issue, change the Clear Hidden Field Values to Don't Clear inside of the Form Settings.

    As result, you will see all form fields and values, regardless of the selected option.

    The '/' is missing because you forgot to add.

    Thank you so much for your help... very simple fixes, but for some reason I could not figure it out. I appreciate your assistance. Another quick question...

    We are using this form to, again, populate menu selections on a 13 week rotating menu. This will be something that around 170 people per week will be submitting. Is there a recommendation for a plan or pricing that we should know about?

    Also, is there a way to change the selections on the form without having to copy and paste into each individual box?
