Sir i working on social media marketing ( SMM ) i want create like this type of form
First my service name - Instagram, Facebook
Second Service Package - like instagram followers , views , Comment
Third - Service Quantity and service divide into 1000 then seen down price auto say for 100 Quantity price $4.50
How we create this types of form please say sir please help when this all features available in your forms we buy your plans

You may ask for the username of the account.

Hello there,
You can use the Form Calculation widget for this.
Add a Number element to your form for the Quantity. Set your Limit Entry if you need to have one.
Use the Form Calculation widget to get the product of Quantity and 0.004 as the Charge.
