I signed up and start drafting and making a new form, suddenly the form become not available and my account was suspended.

I'm not sure why, but happy if you direct me and re activated my account so i can continue using your form.

Your account was auto suspended due to this field on your form:

The collection of login credentials is considered phishing as per our Terms.

I have removed that field from your form and activated your account.

Please do not collect login credentials on forms as it can lead to suspension again.

It was not the intention, I was just plying with the form first, but now I understand and it wont happen again.
