This is the fifth time my account is being suspended. I have adjusted my form as requested to stop this error, however it still locking me out. If I can’t accept payment why do you have that option ? Do I need to remove PayPal period to stop this form locking me out ? Please help me understand my option to avoid this .

I did check your form and found that it is currently suspended as your form is suspected to be used for phishing. Please note that we have an automated anti-phishing filter which scans all forms.It seems our anti-phishing filter marked one of your email as spam which triggered your account suspension.
I have gone ahead and activated your account again. All your forms are back online and should work as expected.
