I want to create a Tale of Contents on my multi page form with anquors that can jump back and forth from the table of contents links to the appropriate pages in the form. I beleive this can be done, but just can't seem to figure it out.

A simple Table of Contents can do. You can combine conditional logic using our forms. Example: http://www.jotformpro.com/form/33135453158957. I use Skip to a page condition for this form.
You can clone this form by following this guide: How to clone an existing form from a URL so you can see how I made this. Let us know if you need any further assistance implementing this to your form.

What if someone does not want the form they have made to be cloned? Is there a way to prevent others from doing so?

Unfortunately, there is no option to prevent other users from cloning your form. If the form URL is known, people can clone your form.
I am not sure if any workaround is possible to stop any form from being cloned. I will speak to my colleagues and get back to you.

What if I purchase the form and have it on my own website server? Could a programmer modify it somehow to keep it protected?

We basically do not have a function to stop someone from cloning your form. If you are after of your form data and information, no worries because someone could not get it from your form. When someone clone a form, he/she will not be able to see you form information, i.e. your email address. Only the design is copied and nothing more.
Furthermore, you may consider using our License Application: JotForm Application. This way, you can take control of the entire Form Builder function, you can disable some of the feature from the system. If you have any further questions with this Application, please direct it firstname.lastname@example.org. Someone from the management should be able to answer your questions regarding this one.
