Inbox Submission not showing names - Showing Submission 1, 2, 3...

    Hello,

    In the inbox, on the left, there is an index of all the submitted forms it used to show the Patient Name for all submission but now it's showing "Submission 1" "Submission 2" etc.

    Hi there,

    Happy to help!

    As your account is HIPAA-Enabled, I am not able to log in and check it myself. As such, I made a clone of your form 201845973678269 and sent through some test submissions in an effort to replicate your issue.

    Each submission came through with the proper Patient Name in the submission data. I did go in and check the Header Settings and each seemed to be in order. On your original form, however, could you please check the following:

    Please ensure that the Patient Name is selected as the the field to use as the Header and that the Hide submission header checkbox is deselected.
