Phone field not showing up on pdf form

I have a simple contact form. Two of the fields are phone fields. The form collects the phone numbers and I see them in submissions, but they don't show up on the Preview pdf or a final downloaded pdf. All the other fields show properly but the phone fields are blank. I've looked at the phone field specs and can't figure out why this one field won't populate on the final pdf.

If you are referring to the Original PDF copy of your form submission I have checked it and it seems that only one phone number field (Phone Number-Cell) on your online form is connected to one of the phone number fields on the original PDF Form.

You can add another field to your online form that you can connect to the other Phone Number fields on the Original PDF Form you have uploaded.
