Emails looks different than forms

I am wondering why when I made a form and inserted an image and questions, they don't come back to me that way? Just the answers. I need to send these to a client who is needing them on file for liability.

Is there something I am missing about not being able to see the whole form (images, questions and answers)?

If you need to send the email with all the text and images from the form to your users, you will need to modify the Autoresponder with the text that you wish to be included. By default, only the answered questions with answers will be included in the email. You can modify the Autoresponder from the Form settings:
