I am a pharmacist and planning to do patient home visits using our ancillary staff, i.e. pharmacy technicians and/or community health workers to do patient clinical intake including medical history and medication list. To that end, I would like to create an intake form that addresses all aspects of the patient admissions as well as the ability to take pictures of the patient prescription bottles for pharmacist review. Would this be possible thru JotForms? I am particularly interested in the ability to take pictures and embed it within the intake form.
