My Jot form isn't Working

I've been trying to use my jot form and I'm unable to do so. I really need this form for my business can someone please explain to me what's going on. I would like this issue fixed immediately.

As my colleague mentioned above, this has to be executed to reactivate the account or have the offensive forms deleted.
