What's the best way to create a form where can fill it out and the submissions will auto-populate into a contract and addendums that I have created?

    I also want to enter one answer and have it populate in multiple fields. IE- entering their name once, and it auto-populates multiple areas that require their name... does it do that?Sent from my Verizon, Samsung Galaxy smartphone
