Thanks for providing the embedded form URL. I checked the form and it seems to be under the account/username mcmule45.
I cleared the forms cache on that account, could you please try re-integrate the form to PayPal once again?
If in case the issue persists, kindly share the console error logs so we can forward it to our developers to check further.
