Can't get to Jotform Link

Hi, I can't get to my jot-form link, it says "Form not Found form is disabled."

It's pre-authrization form with su****t@on****all.com.

It does work with my cuurent email **ng@on****all.com

It is suspended because the form is collecting credit card information. I removed the guilty fields so I can activate your account. Please check it again and let us know if you need further help.
