Account has been suspended

    Not sure why. Says has something to do with phishing prevention?

    Your account was suspended since it was asking for sensitive information (eg: login credentials) which is against our Terms Of Use.

    In order for us to reactivate your account, we have to delete the offending fields. Do let us know if you agree with this.

    Yep I agree, but do you actually have a copy of what the original form
    looks like? I haven't seen it for almost a year so I need to refamiliarise
    myself with it so that I can gauge whether removing the login credentials
    fields is going to impact on what my client needs.

    Please check the screenshot of your form which has fields asking for user's login credential:

    To activate your account again, the "Login Information" section needs to be deleted from your form. Please confirm if we can delete the fields from your form, and we will do the needful.

    I have now deleted the offending fields and reactivated your account.
