THe Shipping Address keep missing from the completed forms I recieved. I know they are being filled out becuase I have been testing the forms myself. When I recieve the emails they are showing blank. Any ideas how I can fix? Its really urgent that this issue be addressed!

I've fixed this. You've made changes to the form, and you forgot to change the email notification as well. Basically, the notification would automatically do the changes to the field tags but once you've modified the default formatting, this function won't work anymore.
Can you please make a test to see if the issue was resolve?
