Information missing from .pdf of submitted form

    On this form:

    There is the following statement that shows up on the form when the person is filling out the form, but does not show up in the .pdf of the completed form that is sent as confirmation.

    10. Individuals renting for personal or event usage are expected to secure "Event Coverage" with a local insurance provider of their choice. Groups and Organizations are expected to secure CCA as an "Additional Insured" to their institutions insurance policy for the dates agree upon in this contract.  

    This statement is important but because it is missing in the .pdf it appears as though it does not exist. Why would this be eliminated from the .pdf?

    Attached you can see what would be page 4 and the aforementioned #10 is absent.

    I have tested a cloned version of your form and all the fields are showing at my end.


    If possible, can you please try creating a new PDF document and attach that to your email?


    Let us know if the issue still remains.
