Questions on if this will fit our healthcare business

    We are a healthcare clinic looking to move some of our services onto an online platform.

    We'd like the ability to do the following.

    Patient learns about medications/services > pics the product he/she likes > is then prompted to fill out forms based on this response (patient data, medical history, etc), we'd like the forms to include a signature button and a way to upload photos (bonus points if they can take them by granting the forms access to their device's camera)>> once forms are complete, they are prompted to pay for their selected options. We'd like to set up recurring payments as well, unsure if that is handled on your end. (we use square)


    Lastly, all of the forms that the patient fills out need to be HIPAA complaint and secure. How do we go about accessing them once submitted?

    The way I see it, you can make use of the Collapse field for the list of services to show and hide the question fields at once, and base on the user's selection: Show-or-hide-multiple-fields-at-once-using-the-section-collapse-field

    Alternatively, you can use the Multiple Choice field for the list of services, then create a conditional logic to hide or show multiple fields: How-to-show-or-hide-fields-base-on-users-answer

    You can create conditions as well to calculate the price based on the user's selection: How-to-insert-text-or-calculation-into-a-field-using-conditional-logic

    Then pass the calculation set up a recurring payment on your form. On this guide, it uses Paypal Personal as an example, but the process is similar to Square: How-to-setup-a-recurring-payment-option
