I’ve created a firm to obtain a testimonial. How do I send it to the person I’m asking to fill out the form and send the completed form back to me

You can copy the form link and directly email it to them using your personal email inbox or from within JotForm.

Here is how you can send the form via email with JotForm: https://www.jotform.com/help/303-how-to-send-a-form-via-email (but there is a limitation of adding 5 email addresses in one email).

You can also assign a form to your patient to fill, but then the patient will need to have an account on JotForm to access the form.

Once they fill and submit the form, you can view the submission on your Form submission page: https://www.jotform.com/help/269-how-to-view-form-submissions

You can also get a notification email with submission data if you set up a notification email in your form: https://www.jotform.com/help/25-setting-up-email-notifications
