Account Lock and Form Disappeared

    My account has been locked due to exceeding the number of failed login. I reset my password and it didn't work, which is why the account is now locked.


    I also had a form that was linked to our website, but the form now shows to be disabled and I cannot reactivate it for continued use. Is there anyway to get this form back again? I don't have a copy of the original form to use for recreation.

    Your account was actually suspended. The reason why this happened is that you built a form to offer loans. This is a practice that goes against our terms of use if you do not build the form the correct way. 

    To unsuspend your account, please provide the following information:

    1. Your website or proof of affiliation to a legitimate financial institution.

    2. The site URL where your form is displayed.

    3. Send these proofs to reviewers@jotform.com with Thread #2903065 in the email subject. 

    4. Once we unsuspend it, you must change in your profile the current email with one with corporate domain to avoid getting flagged again by our phishing detector.

    We will let you know about the status of your account after a thorough review of your proofs.
