No consistency in the questions between published form and form result submitted by email

    Hi guys,

I've just received a form submission from a client and I notice insconsistency in on of the questions.
The question on the form the client answered is different from the report I received in my email. I can see that the question is correct on the JotForm Submission tab but I can't understand why would I receive a different submission by email, if it is all the exact same form.

Can someone help me with that?

I'm seding an image attached for clarification.

It seems you have edited your email alert and that is the reason when you make changes in your form, it is not reflecting in the email alert template.

The best way to solve this problem is to delete the existing email alert template and add a new one. When you add a new email template, it will always include all the questions of form with the exact same label. The following guides should help you:

    Delete email alert: https://www.jotform.com/help/132-how-to-delete-notifications-autoresponder

    Add new email alert: https://www.jotform.com/help/25-setting-up-email-notifications

    Hope this helps.

I did edited the email alerts to include my logo and a bit of text at the bottom, but I haven't edited anything in the email fields. I will have a look at the guides you shared, though. Thanks!!!
