Empty Field on the Tables

    Some of the fields on our form are not showing up on our spreadsheet once the patient completes the form. Thank you.

    Thanks for reaching us.

    Please make sure that you have checked all columns on the Tables. If there are hidden columns, you can not see them even if they are filled. Please check my screencast below:


    Please check the columns and if the issue is not because of this, please provide us the screenshot of the empty field. Here is the guide on How to Post Screenshots to Our Support Forum.

    Yet, I cloned your form and tested it. As a result, all the fields that I filled appeared on the Tables.
