Form Doesn't Load on iPhone But Does on Mac

    Hi Jane,

    I tried accessing your form on an iPhone 11 (version 14). There was no problem with accessing it. However, it seems that your form is private, that's why I was not able to test the form content.

    It first required login:

    I cloned and tested your form on the same device just to see the form comes through successfully, and it did so:
