Problems with signature

My signature is not showing up on some of the records while showing up on other submissions in the same table. Please advise.

Also, when I go to download all pdf's from the submissions table, the zip is not showing all of the files and not showing the names of the files. That just started happening. Prior to when it started, the zip files returned with the file names being the first and last name of the persons in each record

I checked this form (https://www.jotform.com/form/210458732241147) and the signature field is not set to required. Could you please enable the Required feature and let us know if the issue happens again?
