Form not displaying phone field well on phone

This form was displaying phone field off the side (see attached screenshot of MAIN PHONE and CELL PHONE).

I adjusted layout and republished. Seems to be fine now but wanted to let you know.

Only seemed to be an issue on the PHONE fields.
