I have an account with you under jazz journeys that I haven't used in quite awhile but just tested it out on my website at, www.jazzjourneys.org and it's not working. I am getting a 500 - Internal Server Error message.
Can you tell me why this is happening and how to fix it? Additionally, since I haven't use JotForm in quite awhile, would you provide me with a tutorial on how to do so?

Hello Earle,
I checked the contact us form on your website http://www.jazzjourneys.org/contact_us.html and it does not look like a JotForm and also not similar to the Contact us form on your account.
Please check and let us know.
