Hi there,
The top three fields on the VF Corporate Communications | Editorial Input Form are not appearing on submissions even though they are all marked as required AND appear in the Notification email preview. These fields are the most important as they tell us who submitted the form and their contact information.
We need to get this fixed for future submissions, but
also recover who sent the two previous submissions as soon as possible.Thank you for your help!

Hi,
Thank you for reaching out.
The way you set the conditional logic is asking Jotform to show those 'missing fields' only if the condition is met.
Perhaps the way you define the condition needs to be changed.
Please clarify what do you want to achieve with your condition.
Do you want to hide these 'missing' fields until a file is provided in the Upload field?

I have moved your question related to the previous 2 submissions to a separate thread.
I have cloned and fixed your form.
As you can see that the 'missing required fields' are showing correctly:
You just need to remove the invalid 'conditional logic':
