Creating a form and then it deletes

I was working on a new client intake form when jotform popup comes up saying form is disabled. Is this what is to be expected when trying to set up a form?

You have log in form in your account that capture account/login password.

We can only request for consideration to reactivate your account if you comply to the terms and agree to delete the suspected form on the account.
