conditional logic Check-in procedure form

    I want to create a conditional logic Check-in procedure form. 

    Can multiple conditions be used to determine the next questions? If they answered this yes and this yes then this question next.

    Can the results be sent for each individual response (each customer check-in) in a condensed for so that we do not have to review a lot of unnecessary questions that were skipped. Only show me the answered questions and only the the answer selected not all of the options that were not selected.

    If yes then can the results be individually (each customer check-in) exported as a csv file or pdf file, or excel file.

    For the results being sent to you, you can set up and modify what we call a "Notification Email": https://www.jotform.com/help/25-setting-up-email-notifications
