PDF document not showing all form fields

Hi Guys

Can someone please assist me with this form, i really do not know to get this right.
In the next link is the actual form I designed:

The link have 3-4 next button pages. Please help?

However I need this submission form to come out looking like the attached photo.
As I need this letter head to be actively on the first page at least, if possible, it will be awesome to have the letterhead on all the pages of the this submission form.
But when I submit this online form, then it only come out with one pdf page, only with basically less than half the information on that single page.

Please help where do I start?
