No confirmation received for a jot form submission

I completed registration using a jot form for Bigger Than Basketball for my sons. I did not receive a confirmation email for the first registration of my son Issiah Rucker. Please review for that submission and advise?

I cloned it and the autoresponder email is configured properly. If you want to send the autoresponder email to the Confirmation Email field, please update the Recipient and change it to Confirmation Email.
