I am not understanding why my account was suspended? I use you all service for my business. I have appointments and this is an inconvenience to me and my clients. How can I unsuspend my account. Please help, thanks in advance!

Hi,
Your account got suspended by our reviewer team because one of you form is collecting credit card information using basic form fields. We need to remove the form fields before we can activate your account. You can check our terms for more information: https://www.jotform.com/terms/

As per your request, I removed the guilty fields and activated your account. Please check it again and let us know if the issue still persists.
