Form Submission: Why my client cannot submit the form?

I made a test submission on your form, and I was able to submit it successfully. Please note that there are two required fields on your form, and the form cannot be submitted until the required fields are filled. It might be possible that your client missed those fields.

Since you have a long form, I would suggest adding Page Breaks in your form to make it easier for your clients to fill all the fields.
