however once PDF is downloaded it's all misaligned and not effeective to use.

    Additionally, this form New Patient Intake seems the only one we are able to download to PDF. however once downloaded it's all misaligned and not effective to use.     Please try creating a new PDF document in the PDF Editor as shown below.

    Then please tell us if the formatting in the new document is correct or it is the same.
