We are getting bad gateway error when submitting forms

    I am unable to access my forms. I get a bad gateway message. I need my forms for a patient visit today.

    Hi,

    We apologize for the inconvenience this has caused. We had this incident today and our team has managed to fix it. It should be good by now. They are also working on a permanent solution to get this resolve permanently as soon as possible.

    I am still having the same problem. with a form that was completed and
    submitted today, according to my patient. It says "temporarily back end
    unavailable". Please help.

    We are truly sorry. I cleared your form cache, do you mind checking it again this time? If the problem persists, can we make a test submission on your form too if that's okay? I'll also escalate this to our dev team for further investigation.

    It still says "Temporarily back end unavailable" for one of my forms, the
    agreement to Long term controlled substance prescription form.
