Can users book two appointments through one form?

I'm looking to use Jot Form as a solution for COVID-19 Vaccine management.

Is it possible for patients to book two appointments at once on a form? Ideally, since we will be administering the Moderna vaccine we'd like for patients to book the second one 28 days from the first.

Unfortunately, it is currently not possible to select multiple time slots in the appointment field. If you would like your users to select time slots from two different days on the appointment field, they will need to submit the form twice.
