HIPAA: Temporarily Back-End Unavailable

    I am trying to access a form that was submitted by a patient and I get this message - Temporarily Back-end Unavailable. It's only in one of the form areas - Pink Link Medical - FEMALE Patient History Form. What is going on? How can I retrieve the form? I need it for appointment. Thanks.

    I have tried to attach a pdf and it still gives me an error message below asking for a pdf and the message doesn't go away. So I can't attach anything. Very frustrating!
