Not receiving password reset email notification.

I am not being able to "save and continue later" on this application form for the Winston-Salem Foundation Small Grant Application. I am awaiting an email so I can create a new password and I have checked spam and inbox.  

Have you created an account on JotForm since I could not find one linked to lcokernc@gmail.com?

Please also let us know the form URL that you have been provided to fill out. On which email address did you receive this form to fill up?
