    I found that using the form's direct URL seems to display just fine.

    But I was able to see the issue you mentioned when viewed using the embedded form in your website.

    Please try injecting this CSS codes into your form.
    Use this guide on how -How-to-Inject-Custom-CSS-Codes

    @media only screen

    and (min-device-width : 320px)

    and (max-device-width : 480px) {

    .form-all {

    width: 320px !important;

    .form-line {

    padding-left: 5px !important;

    .form-dropdown {

    width: 150px !important;

    Please try this and test again.

    Inform us if there's more we can help you with.

    I took a look at your form via the direct link here as well as on your website but I don't see the form being cut of on my Smartphone.

    What Phone are you viewing this on? What Model Phone is it? It looks like you are using an iPod or iPhone maybe? Is that correct?

    What OS and OS Version are you Using?

    What Browsers and Browser Versions is this happening on?

    Below is my Test Results:

    I tried this in Mobile Chrome, Safari, and also Photon with same results each time on my end.

    iPhone 4S (iOS 7.0.4) : Mobile Safari (latest version)

    Kade, if you scroll up a little, there is a text saying "Load a few clear pictures...". The text is being cut. Somehow I am not able to paste image here but when you see you will be able to figure out.

    By the ways, I am using iPhone 4 with iOS 6.1.3

    Sorry for the delay.

    I just checked your form and I am not able to see the text you mentioned anymore. Did you remove it? Are you satisfied with the current form now? Your form seems to be in good standing without the free text field however we can always try to dig more and find out a fix if you're going to put it back. Just please let us know anytime, we just can't proceed on testing without the problematic text field.

    Thank you so much!
