Apologize for the Inconvenience this may have caused.
I inspected the form and the broken images are connected to the account jwyre88, which no longer exists in our system. I checked the history logs on your account, and I saw that the username of your account was never changed, so I assume that you cloned that form.
I can only suggest you remove the broken images from the Image Slider widget and re-upload them.

That's really strange, bc I added those photos to my new account, not the jwyre88. And up until like yesterday they were still there. But ok I will re-upload them.
Is there anything else on that form that is still connected to the deleted account?

The header logo was also connected to the account jwyre88, but I saw that you already changed it.
Please re-upload the slider images as well.
