Fill the form in on pphcare.co.uk /employment and send it no longer hits my in box and if i go to my jotform account the information is empty can you help thanks Philip

We are truly sorry for the inconvenience caused. The problem is now fixed. You should now be able to see your form submissions data as expected. This was due to a data replication issue between our servers recently. It's all good now.
