How do I add categories back into the form?

    When I initially created form there were different categories for membership dues. After I added the Square payment, the categories went away and I don't know how to add them back. Please advise. Thanks

    Please note the categories can be shown only when the payment type on the payment field is "Sell products" or" Sell subscriptions".


    Currently, you have set this up as "User-defined amount".
