My survey is currently locked

    I recently created a new jot form and today I noticed it's locked. Would someone please advise what needs to be done to reactivate it?

    As checked, your account has been suspended as this form: CC_authorization 2 contains plain text fields that collect Credit Card information. It is considered phishing and violates our Terms of Use.


    You agree that Jotform, Inc. may terminate your Jotform Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    You can use one of the Payment Integrations available with Jotform if you are to collect payments using your forms. Transactions with these integrations are secured instead of using plain text fields.

    I have deleted the said fields from the form and reactivated the account. Please avoid adding similar fields in the future to prevent account suspension.
