I would like to put an email address box in one of my forms so that when the form is submitted it goes to;
A) the person who has submitted it
B) the correct email address that all forms go to
C) another persons email address that is put into the form
I have managed to do A and B but cannot seem to do C

For "C)", have you added another email field in form?
You need to add another "Form Auto-responder" email alert in form and select the email field of form as the "Recipient Email". Please check the screenshot below:
