We're not sure why, we're using the same link as before. We have a number of outstanding forms, so we can't even tell whose it is. Just wondering if there might be an easy answer? If we made a small edit to the JotForm, would the url we send our patients not remain the same? Thank you!
UPDATE: The forms are suddenly showing the responses now on the ones that weren't before, but is there a way to find out why it happened in case it happens again?
