3 Form Fields Not Showing in Submission Data

    My coworkers created a form and when viewing the form all fields are accurate. However, the data received is missing 3 new form fields that were added after the initial creation of the form.

    I attempted deleting the 3 fields from the form that were not providing data in the submission area, and recreated them fresh (in case that had been created by duplicating a previous form field and there was for some reason a glitch that stuck in the coding). However, this did not help.

    Also, in the submission area where you can select columns to show, it does not have an option to select any of these 3 new form fields.

    These are the 3 fields that do not offer data in the submission area, but if you view the link above with the form you will see they are in the form.

    Thank you so much! I could not hear any audio with the video... but I do see where that data is displaying now, thank you! However, is there a way to get that data to display when the form submission is downloaded? When a Jotform comes in, we download the submission (in PDF), then upload it to the patient's electronic folder. However, when we download that form, that is when that information (see first question) goes missing... and we cannot figure out how to fix that... Thanks!

    It looks like the PDF document template is no longer updated. Please try deleting the existing PDF template and create a new one. That would generate a new PDF document with all the updated fields added to it. Here's how:
