When I look at my form on a computer it looks great, buy when I open the web page on my iphone it doesnt look right, It goes from a square form to a long recangular form. any sugestions ??

That is how the form is rendered on mobile since the form itself is optimized for mobile views. If you have embedded the form to your website, let us know the website URL so we can check it here.

Just like what my colleague Elton have mentioned, that is how the form works when viewed using mobile devices. You can further customize the look of the form by injecting another custom CSS codes that would only work for mobile. Media Queries: Media Queries for Standard Devices | CSS-Tricks
We can help you to redesign the form using the method. But first, we need to know how you'd like it to be. Not sure if the same format when viewed using a computer is possible. That may look too tiny.
