Please review this guide Adding-Form-to-Facebook. Try to follow the guide step by step so that you can properly add your form to facebook.
From what I can see in your facebook page so far -- it is likely that you were not able to do it properly as described from step #3 in the guide3. There are two ways you can choose to display your form on your Facbook page. The first makes it available to everyone (Public Content), while the second makes it available only to fans, or users who liked your Facebook page (Fan Content).4. Select whichever method of display is appropriate by clicking on edit button. Select "HTML" as your editor, paste your Facebook form code from JotForm, and saveThere may be some security issues with old versions of Internet Explorer (7 & 8) for non-secured protocols (http). You should take note of the error after pasting a non-secure form. If your form is not showing up in Internet Explorer on Facebook due to this issue, make your non-secure form to secure by changing http://form or https://www to https://secure in the form URL found in your Facebook Form Code. Example:<iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="https://secure.jotform.com/form/21357249698871" frameborder="0" style="width:100%; height:457px; border:none;" scrolling="no"></iframe>
It's highly recommended that you use "https" to avoid any security issues on the page since Facebook requires secure protocol.
