    How to create forms for a single page?

    hi all,

     

    would like to know if it is  possible to set a kind of form with three different page. customer can choose the one he wants to submit.

    Hi,

    I would suggest creating these three (3) forms to accomplish this one. Then create another form (4th form) that will server as your base form. In this base form, use the following tools:

    1. Radio Button or Checkbox fields to ask for what form they would like to submit

    2. Free Text HTML field to embed the three (3) forms

    4. Conditional Logic to show the form per selection on the radio button or checkbox field.

    Example form: http://www.jotform.co/form/33213076186855. This is the base form.

     

    Form 1: http://form.jotformpro.com/form/33212218516950

    Form 2: http://form.jotform.co/form/33213151992854

    Form 3: http://form.jotform.co/form/33212844574858

    All these forms are embedded to the base form using the Free Text HTML field. I use iFrame Method: Getting the Form iFrame Code

     

    You can simply clone this form to see the overall setup, please follow this guide: How to clone an existing form from a URL.

    ok. is it possible to link all the three forms to an only one thank you page, like the one i set here : http://form.jotformpro.com/form/33175405094957 . with the same ID number for the 3 forms

    Hello Jaures,

    I am not sure if I have understood your requirement correctly. Do you want to display the below mentioned thank you message as you have currently in your form:

    If yes, it is possible to achieve this.  You can create one new webpage in your own server and use that webpage in your custom thank you URL in all three forms you have. Here is a guide which should help you:  https://www.jotform.com/help/211-Setting-Up-the-Thank-You-Page

    Sharing the input value to your custom thank you page will be a challenge but can be achieved with some custom programming.
